IASTM, the acronym for Instrument Assisted Soft Tissue Mobilization, was originally conceived and developed by several top athletes who used everyday objects to help them break up their own myofacial and muscular adhesions, IASTM is an innovative, evidence-based form of instrument-assisted soft tissue mobilization that expounds on the ancient practice of gua sha. This modern modality has enabled manual therapists to break down scar tissue and fascial restrictions effectively, allowing for greater range of motion, less pain, and quicker post event recovery. Specially designed stainless steel, or other hard material instruments are utilized to specifically detect and treat areas exhibiting soft tissue fibrosis or chronic inflammation. Popular variations of this modality include Graston Technique® and Técnica Gavilán. IASTM tools for removing adhesions Instruments used with this modality break down fascial restrictions and scar tissue very effectively. Their ergonomic design provide practitioners with the ability to locate soft tissue restrictions and helps them treat the affected area with the appropriate amount of pressure. By introducing controlled micro-trauma to affected soft tissue structures, IASTM causes the stimulation of a local inflammatory response. This micro-trauma initiates re-absorption of inappropriate fibrosis or excessive scar tissue by the muscle and facilitates a cascade of healing activities which result in a re-organization of affected soft tissue structures. Surgical adhesions, or those which may have developed as a result of immobilization, repeated strain or other mechanisms within the soft tissue, are broken down easily allowing full functional restoration to occur. Other indications of where IASTM may be useful are soft tissue that exhibits limited motion, pain during motion, motor control issues, and muscle recruitment issues. Several studies have addressed the benefits of IASTM at the cellular level. Among the benefits are increased fibroblast proliferation, reduction in scar tissue, increased vascular response, and the remodeling of unorganized collagen fiber matrix following IASTM application.
